Refactor is an authoring tool. Teams write the skill and the scripts beside it, publish a version, and sharpen it when runs come back.

EDIT vs RUN vs VERIFY

The catastrophic confusion is editing the Process for all future work while believing you are tweaking one result. The console keeps mode distinct:

• EDIT — change the definition (gated)
• RUN — execute against an engagement / subject
• VERIFY — judge a specific run's output

Publish

Publishing freezes a process version. Production runs pin that version until you publish again (or deliberately re-pin). Keep the writing legible — future humans and future agents both read it.
